The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

   

Payments: Unless other arrangements are approved by us in writing, the patient portion of your account is due and payable the day services are rendered.

Divorce:  The parent authorizing treatment for a child will be the parent responsible for those subsequent charges.  If the divorce decree requires the other parent to pay all or part of the treatment costs, it is the authorizing parent's responsibility to collect from the other parent.

Returned Checks:  There is a fee of thirty dollars ($30.00) for any checks returned by the bank.

No Show Fee:  Our patients need the time they have reserved exclusively with the provider, to maintain optimal oral health.  We charge a fifty dollar ($50.00) fee to patients who do not show up on time for a reserved appointment, or cancel with less than forty-eight (48) hours notice.  This is to curtail "No Shows", and make sure patients who need to be seen are seen in a timely manner.  We also have patients who are in need of immediate emergency treatment.  Those patients can be placed on our schedule if we are given a forty-eight (48) hour notice that you need to reschedule.  Patients with three (3) missed reserved appointments will be asked to transfer their records to another doctor.

Waiver of Confidentiality:  You understand if this account is submitted to an attorney or collection agency, if we have to litigate in court, or if your past due status is reported to a credit reporting agency, the fact that you have received treatment in this office may become a matter of public record.

Past Due Accounts:  If you fail to pay your account in full within ninety (90) days following your office visit, we will refer your account to a COLLECTION AGENCY.  You shall be responsible for paying the fee that the collection agency charges for collection of your debt.  The amount added is forty percent (40%) of your debt.   You understand and agree to pay that fee.  Also, please understand that you are still responsible for any court cost and recovery costs associated with collection of your debt.  In case of suit, you agree the venue shall be in Garland County, Arkansas.  In order for us to service you account or to collect any amounts you may owe us, you agree we may call you at any telephone number associated with your account, including wireless telephone numbers, which could result in charges to you.  We may also communicate with you by sending text messages or emails to your wireless number or email address.  Methods of contact may include using a prerecorded/artificial voice and/or the use of an automated dialing device.  These authorizations shall remain in effect until withdrawn by you in writing to us and/or any others to which authorization has been extended.  I have read this disclosure and agree that "your office or agent" may contact me as described above.

Dental Insurance

Insurance Benefits are a contract between you and your insurance company.  Insurance coverage is filed as a courtesy for you.  Your insurance benefits are decided by you and/or your employer and your insurance carrier.  We will do our best to ESTIMATE your benefits, but DO NOT GUARANTEE coverage or benefits. Insurance benefits will be verified before your visit and all deductibles and copays are due at the time of service unless prior arrangements have been made.
